MRI Liver Elastography (MRE): The Fuller Picture

Along the way I learnt about one more liver test, and it deserves a mention here. MRI Liver Elastography, or MRE, is a non-invasive liver test that needs far more awareness than it has today. The idea is beautifully simple. You lie inside a standard MRI machine while a small vibration pad rests on the abdomen, sending gentle, painless vibrations into the liver. The MRI watches how those waves travel through the tissue and converts them into a colour image of the entire organ, soft areas here, stiffer areas there, a stiffness map of almost the whole liver, produced in a single study. MRE also reports a Liver Stiffness Measurement (LSM) for the liver as a whole.

The strengths of MRE are considerable: it examines nearly the entire liver rather than any single point, it is painless and involves no radiation, and it delivers reliable results regardless of body size or fluid in the abdomen. Its limitations are practical ones: the study takes longer and requires a full MRI setup fitted with special equipment, which is why, at the time of writing this book, MRE is done at only a select few diagnostic centres. Availability is a matter of time, not of science, as more centres install the equipment and more doctors ask for the test, MRE will reach more cities. And the final word is simple: let your doctor decide which liver test is ideal for you.
