Ultrasound of the Liver: The First Window

Chapter 7 · Vihang Rahul Vasa

The humble ultrasound is usually the first look anyone ever gets inside their liver. The technology is wonderfully gentle: a hand-held probe glides over your abdomen with a little gel, sending harmless sound waves into the body. The waves bounce back from the tissues, and a computer converts those echoes into a live, moving image of the liver on the screen: its size, its shape, its texture, and whether fat is visible within it. The entire examination takes a few minutes, involves no needles, no radiation, and no pain, and requires no recovery time at all.

Its advantages explain why it is everywhere: ultrasound is available at almost every diagnostic centre and hospital, from metropolitan cities to small towns; it is quick, easy, completely safe, and can be repeated as often as your doctor needs. For countless people, including many living with diabetes, a routine ultrasound is the first test that ever raises the flag: “there is fat in the liver.” That single sentence, caught early, has changed the direction of many lives, including mine.

Its limitations are equally honest: an ultrasound produces an image, not a measurement. It can tell your doctor that fat is visible, but only approximately how much; it cannot measure the stiffness of the liver, and early scarring can escape it entirely. The quality of the result also depends on the skill of the person performing it and on body build. None of this makes it a lesser test: it makes it a first test: the universal starting point of every liver journey. As always, let your doctor decide which liver test is ideal for you, and when.
